Where Does Hospice Care Take Place?

Home

Most hospice patients receive care at home. A dedicated nurse case manager coordinates care alongside social workers, chaplains, and certified nursing aides. Visit frequency and services are tailored to each patient’s needs and may change over time.

Long Term Care Facility

For patients living in skilled nursing or assisted living communities, hospice care is provided in collaboration with facility staff. The hospice team works closely with caregivers to ensure seamless coordination, comfort, and continuity of care.

Inpatient Care Center

When symptoms become too complex to manage at home, inpatient hospice care may be needed. Pikes Peak Hospice & Palliative Care is the only hospice provider in the region with a licensed Inpatient Care Center, offering short-term, 24/7 expert care in a comfortable, home-like setting focused on intensive symptom management and stabilization.
